Needed to Register – You must present all of the following information to the DOT at registration:

  • A completed  Application for Certificate of Title & Registration of a Vehicle (SFN 2872)
  • You must provide proof of your legal name ie: Valid driver’s license, Non-driver ID card, Any document that confirms your  legal identity
  • Your signed vehicle title and a 5% excise tax as required.
  • If you do not have the title because you’re financing your vehicle provide the following:
  • Name and address of the lien holder
  • Loan account number.
  • Out-of-state vehicle registration
  • Proof that you have paid or remitted the 5% excise tax
  • If you’re registering a new vehicle, you will require a Certificate or Origin and a Odometer Reading statement from the dealer
  • Be prepared to pay all registration fees

How to Write

Step 1 – Confirmation of Amount of Sale – Enter the following in the proper format:

  • Enter the worded format of the sale price of the vehicle
  • Enter the date of the agreement in mm/dd/yyyy format

Step 2 – Buyer’s Information – Enter all information into the required fields on the form:

  • Print or Type the Buyer’s Legal Name
  • Check the applicable box in the next field to the right on the form

  • Zip Code

Step 3 – Vehicle Description – Provide all information in the required fields:

  • Year
  • Make
  • Model
  • Body Style
  • Vehicle Identification Number (VIN)
  • Color
  • Fuel Type
  • Weight
  • If the certificate of title cannot be provided, enter a brief explanation as to why in the line provided on the form

Step 4 – Seller’s Information – Provide all required information as follows:

  • Print or Type the Seller’s Name
  • Telephone number
  • Address
  • City
  • State
  • Zip Code
  • If the seller has any comments or disclosures, enter them on the line provided on the form

Step 5 – Notarization – The seller must review the information on the form, for the purpose of notarization and enter any possible lien information if needed. The notary public will then complete the form and affix the state seal

  • Make copies of the document for record keeping
  • Provide the original to the buyer
