The Employee of the Month Nomination Form is a template which providing a method to vote for an Employee who someone believes should receive the distinction of Employee of the Month with his or her Employer. Typically, entities who choose to honor employees with this distinction will have a voting procedure set in place. This form will complement, or even improve upon, such voting procedures by providing a uniform written ballot for Employee Voters. This is in the best interest of the Employer as a structured form will be easier to review and tally.
How To Write
Step 1 – You may download this form as either a PDF document or as a Word document. To do so, simply select either of these buttons on the right hand side of the page then save it to your computer.
Step 2 – Locate the blank line labeled “Today’s Date” at the top right hand corner of this page.
Step 3 – On the blank line following the words “Name of Nominated Employee,” enter the Full Name of the individual you are voting for.
Step 4 – On the blank line after the words “Nominated Employee Position Title,” report the official position the person you are voting for holds in this Company.
Step 5 – Next, report how many years the Employee you are voting for has worked for this Company on the blank line labeled “Number of Years Working for Employer.”
Step 6 – On the blank line labeled “Name of Employer,” report the Name of the Company where the nominated Employee works.
Step 7 – Report the Phone Number of the Company where the Nominated Employee works on the line labeled “Employer Phone Number.”
Step 8 – The second half of this page will contain several blank lines. You must provide at least one or two reasons why you believe the Employee you are voting for should be Employee of the Month here.
Step 9 – On the blank line labeled “Your Name,” then Print your Full Name. Below this on the blank line labeled “Signature,” Sign your Name.
Step 10 – Submit this Nomination to the appropriate office or drop box according the company policy set forth in this voting procedure.
