The Hawaii Motor Vehicle Bill of Sale Form CSLVMR 40 is a legal document that will establish the sale and purchase of a motor vehicle in the state of Hawaii. This document will also provide evidence that there has been a change of ownership and will provide required odometer information. This form will require notarization. Do not provide any signatures prior to presence of a notary public to witness the signatures as required.
Register a Vehicle in Hawaii
In the state of Hawaii, state law requires that motor vehicle owners must register their vehicle with the Hawaii Department of Motor Vehicles before taking it on the road. Both new and used vehicles must be registered, regardless of whether they are purchased from an individual or any dealer. As well, new Hawaii residents who have registered their vehicle in another state, must register their vehicle in Hawaii even if only an extended stay visit is all that is intended. If this is the case, an will be required. If this is an initial vehicle registration it must be done in person. Registration renewals must be done annually, either in person, by mail or online. The vehicle must be registered in the county in which the vehicle is being operated.

If you’ve purchased a vehicle with a Hawaii dealership, the car dealer will do all of the paperwork to get your vehicle titled and licensed. They will also collect your vehicle registration fees so be prepared to pay these fees while you’re closing your car deal.
If purchasing a vehicle from an individual you must provide the following:
- The seller must sign, date, and write the mileage on the back of the Hawaii car title.
- Sign and date the title yourself.
- Get a bill of sale as a receipt
- Proof of Auto Insurance
- Take the title, the most recent certificate of registration, and the state of Hawaii safety inspection certificate to your at local DMV all together.

Step 1 – Amount of Sale – Enter the following in the first two lines of the document
- Dollar amount in words
- Dollar amount in numbers

- License Plate Number
- Vehicle Identification Number (VIN)
- Make, Model and Year of the vehicle being sold and purchased
- Body Type (ie: SUV, 4 Dr Sedan etc.)
Step 3 – Seller Information – Provide the seller’s information as follows:
- Seller’s Name
- Physical Address
- Zip Code
- If there is a lien holder, place the lien holder’s information as opposed to the seller’s information
Step 4 – Odometer – Provide the odometer reading in the line provided, in numbers without 10ths miles

Step 5 – Signatures – If all parties who will provide signatures on the document are present before a notary public, provide the following:
- Date the signatures on the document in dd.mm/yyyy format
- Signature of Seller
- Printed Name of Seller
- Again, Signature of Seller

- Physical Address of Seller
- Zip Code
Step 6 – Notarization – The remainder of this document will be completed by the notary public as requires, they will also affix their state seal
