The Missouri Limited Power of Attorney is a type of form that a person can use if they need representation for a specific transaction or a limited period of time. For instance if there is a real estate transaction for which you are going to be away, you can appoint a person to sign on your behalf. There also may be an instance where you need someone to handle your finances for a short period of time while you are away.
How to Write
1 – The Limited Authority Appointment Form Should Be Obtained Using This Page
Review the form to make sure you understand it. Download the document and begin by filling out your name and address and your agent’s name and address.
2 – Principal Information Should Be Clearly Presented
The Name of the Principal (First Name, Middle Name, Last Name, Title/Suffix) should be entered on the first blank space of this statement.
There will be a blank line immediately following the Principal’s Name you have just entered. Make sure to enter the Principal’s Social Security Number on this line.
3 – The Attorney-in-Fact Must Be Documented Accurately
The next available blank line will require the First Name, Middle Name, Last Name, and Title/Suffix of the Attorney-in-Fact being designated with the Limited Principal Authority being conferred through this paperwork.Once you have identified the Attorney-in-Fact, enter his or her Full Address and up-to-date Phone Number on the next blank line. Note that two separate areas have been labeled on this line so that each of these items may be neatly presented.
4 – The Limited Powers Available To The Agent Will Need To Be Described
This paragraph will not only identify both the Principal and the Attorney-in-Fact, it will also seek a definition to the Limited Principal Power(s) the Principal wishes to appoint the Attorney-in-Fact with. This definition of Power should be documented on the numbered blank lines following the words “…Only The Following Acts On My Behalf.” There will be three blank lines, numbered one through three, provided for this purpose but, if more room is needed for a complete definition of the Limited Powers being designated, then you may continue the definition on an attachment.
5 – Principal Approval Will Be Provided Through The Principal Signature
At end of this form, the Principal will need to report the Two-Digit Calendar Day, Month, and Two-Digit Year he or she is signing this document on the blank lines after the words “Signed This…”The Principal must sign his or her Name on the same Date reported above. The blank line labeled “Signature” must bear the Principal’s signature to execute this form.
